Coaching Agreement, Terms & Conditions
This agreement outlines both our responsibilities in this powerful relationship. I want you to be familiar with policies/procedures so that things can run smoothly between us and there are no misunderstandings later on.
Please read this over and if everything is fine, indicate your agreement and submit at the bottom. Of course, please let me know if you have any questions or concerns!
I have positive expectations for a coaching relationship that helps you to create the creative life YOU want to live. To partner together professionally, I want you to be familiar with the following guidelines.
Terms of Coaching:
I invite you to think of Makers Megaphone Coaching as a process. Many people create change for themselves in a short time. However, to refine and sustain the change for you and your business may take several months. Although not binding, I would ask that you expect our Coaching relationship to last at least 4-8 weeks.
The Makers Megaphone Coaching fee is payable to Makers Megaphone LLC. If you pay by credit card online, you will be charged automatically upon purchase. You have the option to pay in full up-front, or weekly during the duration of our time working together. If you’ve chosen to pay weekly, you will be charged once a week until the complete payment has been made.
At the time of your session, call the conference line at the provided number (unless we make other arrangement). Please call from a quiet place to concentrate on our work together and try to spend our session focused on listening, sharing, and learning. You’ll get much more value. Please be on time for each meeting. Before each session, complete the Makers Megaphone Weekly Progress Form and email to me at (firstname.lastname@example.org) at least 24 hours before our session.
Client Days and Hours:
My office hours are Monday through Thursday 9 AM- 5 PM CST. If you send me an e-mail or text after 5 PM, I’ll most likely respond the next day. If you send me something via email or text on Thursday late day or night, I’ll most likely respond on Monday or Tuesday.
Coaching Weeks and Off Weeks:
We will work together three weeks of the month and have one week off a month for integration of the work we are doing.
Emailing and Texting Between Sessions:
I understand that you might have questions, challenges, successes or inquiries in between our sessions that you would like to connect with me about. Please know that if it is a quick question or inquiry that requires a simple response, I would be delighted to help you and will try to reply during my office hours within 24 hours. If it requires more time or is a more complicated answer I will usually say “let’s save it until our next meeting” so that I can better help you and not short-change you. Thanks for understanding.
Email me at any time at email@example.com, or text me at 917-723-0098 to reach me with ideas, problems, questions, etc.
If you need to reschedule our appointment, please give me 24 hours notice. If you have an emergency or illness, you can reschedule with less than 24 hours notice. If you do not show up for a scheduled call, for any reason, and provide no notice (latest notice can be five minutes past scheduled session start time), we will not make up that time and you will be charged for the session. If I need to reschedule, I will give you at least 24 hours notice as well, barring an emergency or illness.
If I ever say or do anything that upsets you or doesn't feel right, please bring it to my attention so that we can resolve it as soon as possible. My objective is to have a coaching relationship that is fully open, honest, real and trusting in our communication styles. We should both realize that communication via telephone or email entails extra challenges since we can not see body language, facial expressions, etc. Therefore we give each other plenty of latitude, and promptly ask for clarification if there is a mis-connection.
I recognize that you may have and choose to share the following: future plans, business affairs, financial information, job information, goals, information related to relationships, other personal information and other proprietary information. I will not, at any time, either directly or indirectly use any information for my own benefit, disclose, or communicate, in any manner, any information to any third party. I will also not divulge that we are in a consulting relationship to anyone, as a courtesy for your privacy, unless you give me permission to do so. You may, however, feel free to tell others that we are working together if you like.
Nondisclosure of Coaching Materials:
Materials that I give you in the course of our work together is proprietary, copyrighted, and developed specifically for Makers Megaphone LLC. You hereby agree that such proprietary material is solely for your own personal use for the purpose of improving your business. Any disclosure to a third party is strictly prohibited. Fair enough?
During our time together and at the completion of your program, I will ask you to write a few comments about our work together in the form of an evaluation. It helps me to see what you liked about the process, what results you received overall and anything else you’d like to add. I find this extremely valuable and now make it a consistent part of working with each client. I thank you in advance for this. Often, I ask clients if I can use these comments in the form of a client testimonial to share with others who are thinking of taking the next step in their business. You’ll let me know at that time if that’s acceptable.
Disclaimer of Liability:
Client (You) hereby employs Ashley Heyburn as Advisor for the purpose of advising and counseling the client with respect to his/her professional development and setting and achieving goals. Ashley Heyburn has experiences in such matters and agrees to render such advising services. Client understands and Ashley Heyburn agrees that she is not an employment agent, a business manager, a financial analyst, an accountant, a lawyer and that she has not promised, shall not be obligated to, and will not: (1) procure or attempt to procure any employment, business or sales for the Client: (2) act as a therapist providing psychological counseling, psychoanalysis, behavioral therapy or any other medical advice.
As a coaching client, I agree to receive periodic e-newsletter updates from Makers Megaphone Coaching and understand I may unsubscribe from these at any time.
Coach (Ashley Heyburn)
I agree to serve as your Coach - to partner with you to identify and achieve your personal and/or professional goals.
As your Coach, I cannot guarantee results. You will create powerful results by having the courage and determination to forward the action in your life.
During the time we spend together in our coaching sessions, I will devote my time, thoughts, and energy to you, exclusively. In between our sessions, I may not be instantly available, as I may be attending to others, or myself. I will however, always attempt to be available during my office hours within 24 hours.
I am a Coach, not a psychotherapist or physician, and I am not trained in diagnosing psychological or medical conditions. If any issues come up for you that should be handled by a licensed therapist or physician, I insist that you must attend to your health by contacting the appropriate professional.
As your Coach, I will bring attentive listening, understanding, belief in you and commitment to your success. You can expect me to challenge you, offer fresh perspectives, make requests (including assigning homework), acknowledge your wins, and guarantee utmost confidentiality (to the fullest extent of the law, and so long as I don't fear for your or another's safety) in the powerful, sacred relationship.
I am motivated and committed to taking action on my determined personal and professional goals. I realize that anything less than my intentional full participation will not lead to my success.
I accept full responsibility for myself and any actions I take that might result from Makers Megaphone Coaching.
I am under the care of a physician and healthy enough to engage in coaching.
I can financially afford the Makers Megaphone Coaching fee at this time. If I did not pay in full, I agree to pay (or be auto-charged) weekly for my sessions. I agree that ultimately, it is my responsibility that my coach gets paid for the services I use.
I agree to honor my scheduled session times.
I have read and agree with the Coaching Guidelines and the Coaching Agreement.
